Medical Information

Policy on supporting pupils with medical conditions

Medical Information and Care Plans

If your child has a medical condition that school needs to be made aware of then please complete the Medical Information Form below providing as much detail as possible. Where a student has a severe medical issue then please contact the medical lead Miss Naomi Thomson on 01736 753009 or email enquiries@hayle.tpacademytrust.org

If your child has a Health Care Plan written by a medical professional, please email this to the above email for us to add to our system.

Administering Medication

Occasionally, you may wish for school to administer medicine to your child.  In order for us to do this, please complete the consent form (FORM 1) which authorises school to administer the medicine and also do note the following important points:   

  • All medicines must be clearly labelled and handed in to reception for collection by our first aid team.  
  • Authorisation forms will also be held at reception for you to complete when handing in medicines.  Without this consent form being completed, we will unfortunately not be able to administer medications.

Please be aware that medicines should only be taken in school when essential.  Where clinically possible, medicines should be prescribed in dose frequencies which enable them to be taken outside school hours. 

A consent form will also need to be completed for authorisation to administer over the counter medicines such as paracetamol or hay fever medication. The school is not able to provide over the counter medicine; this must be provided by yourself as parent/ carer and will be stored safely in school for your child only.   

This authorisation will then last for the year but will need a new consent form at the start of each academic year.  Please note that these medications will not be administered before 12 midday to ensure the timings of any medications already given are safe to administer.

Please also do note that students are not permitted to carry any medications in their school bag (other than a prescribed EpiPen or asthma inhaler – see below); this is outlined in our banned items list (please refer to our behaviour policy). This is to ensure the health and safety of all students as part of our duty of care to all students. All medicine, including over the counter medicines such as paracetamol and ibuprofen, must be handed into the school for administration with a completed consent form.

FORM 1: Authorisation for administering medicines in school

If your child self manages their asthma and carries their own inhaler or must carry an EpiPen, please complete the consent form (FORM 2) which gives notification of them carrying this medicine and your permission for them to carry and self-administer these.

FORM 2: Authorisation for carrying and administering an Asthma Inhaler or EpiPen

Medical conditions and medication on residential trips and visits

If your child is due to attend a residential trip or visit and you require the school staff to administer medication or require your child to administer their own medication (EpiPen, asthma inhaler, insulin or medicated creams), please complete the consent form (FORM 3). Form 3 will also need to used to inform the Trip Leader about any dietary information for your child. Once complete, please e-mail it to medicalform@hayle.tpacademytrust.org, email or hand in to the Trip Lead, or hand into the reception front desk.

Please note that Form 3 will need to be completed for each individual trip that your child may attend.

FORM 3: Authorisation for carrying and administering medication for residential trips and visits

Crutches/Wheelchairs/Arm Casts/Slings

If your child will be coming into school on crutches, in a wheelchair or has an arm cast/sling please inform first aid at the earliest convenience. A risk assessment will need to be completed prior to your child’s return to school.

Health advice messaging service for families in Cornwall and the Isles of Scilly:

Cornwall Health have launched an enhanced offer for young people and parents / carers to contact Health Visitors and School Nurses in Cornwall and the Isles of Scilly.

Users can send a message, anonymously if they wish, to Health Visiting and School Nursing to get confidential help and advice about a range of health concerns, including:

Sleep
Child development
Support with behaviour
Toileting
Feeding and nutrition
Keeping safe and healthy
Childhood illnesses and support with medical conditions at school
Emotional, mental health and wellbeing
Relationship
Smoking and vaping
Drugs and alcohol

The messaging service is available Monday to Friday (excluding bank holidays) from 9am to 5pm. When a message is sent to the numbers, an automated reply confirms the message has been received. A trained healthcare professional will reply to the message within 48 hours (within office time).
